THE DEPOSIT PAYMENT IS NON-REFUNDABLE. Due to limited schedule availability, a deposit payment is required for all NEW patients at the time of booking to secure a spot. The deposit will contribute to your balance due at time of service; it will NOT be refunded due to cancellations or no shows, it will be forfeited. All major credit cards, including HSA and FSA cards, are accepted.

CANCELLATION POLICY: 24 hours notice for any cancellation is required. Cancellations made with less than 24 hours’ notice are subject to a 50% visit fee. If you do not show up for your appointment without any prior notification (no show), you will be charged 100% of the visit fee. This fee will NOT be refunded or applied to future visits.
